What's the quickest way to house train my puppy?

Crate training is without a doubt the best way to get across the potty routine.  So, what size crate -- one large enough for the puppy to stand and turn around.  Next, place in the crate bedding a toy or two but nothing that is made from paper.  It is rare that a puppy doesn’t immediately love their crate home, and an added plus that they prefer not to go to the bathroom in their home.  How long can a pup stay in the crate before time to go outside? 

The rule is one hour of crate time for every month of age.  This means that a 16 week old pup should go out every four hours.  It is also a good idea to go outside after meals, play or when the puppy first wakes up.
